In-Person Appointment

Policy

Thank you for choosing Lifeline Academy for your Cardiopulmonary Resuscitation (CPR) and Community Based Residential Facility (CBRF) training. Once registration is complete you will receive a confirmation email that your seat has been confirmed. Please check your inbox as well as your spam. You will also receive an additional email from info@mylifelineacademy.com regarding additional information you will need to know for the day of the course.


A $25 non-refundable deposit is required for each registered course. Deposits are also non-transferrable.
 
Lifeline Academy understands that changes in schedules and unplanned events occur. In the event you need to cancel or reschedule, please contact us 24 hours prior to the start of your registered course. If you have physical condition or limitations, please notify us as soon as possible so that we can accommodate your needs.

Please note you will need to create an account with the American Heart Association and pay a fee for the online portion of the course. If you are having difficulty with the American Heart Association website, please contact them directly at 1-888-277-5463. 


* Please bring a copy of the completion of the online learning module to the in-person skills check. Failure to provide a copy of completion will result in cancellation of the in-person skills check-off.

Community Based Residential Facility (CBRF)courses.

Upon successful completion of the registered course(s), you will be added to the Wisconsin Community Based Treatment and Training Registry (CBRF Courses only) within twenty-four to forty-eight business hours.
